A community-based residential services organization is seeking a Bilingual Wellness Coach in the Bronx, NY. This role supports residents with case management, community referrals, and day-to-day wellness services in a residential setting. This opportunity is for a Bilingual Wellness Coach, Bronx.
About the Opportunity:
- Shift: Day shift
- Schedule: Sunday through Thursday, with one day from 10:00 AM to 6:00 PM
- Hours: 9:00 AM to 5:00 PM
- Setting: Onsite
Responsibilities:
- Provide case management support to an assigned caseload of residents
- Assist clients with access to medical, behavioral health, substance use, vocational, food, clothing, and public benefit services
- Conduct follow-up, document services, and maintain accurate case notes in compliance with agency standards
- Accompany residents to appointments and support continuity of care through referrals and coordination with service providers
- Support daily living skills, community integration, and other resident needs in a residential program setting
Qualifications:
- Bachelor’s degree
- 3+ years of experience working with a behavioral health population
- Working knowledge of email, Microsoft Word, Excel, PowerPoint, and Windows operating systems
Desired Skills:
- Bilingual English/Spanish communication skills
- Experience with substance use, mental health, or homelessness services
- Experience in group work, tenant organization, or community outreach
- Experience working in a residential or supportive housing setting
- Strong interpersonal skills, professionalism, punctuality, and reliability
