A Massachusetts-based health services network is actively seeking a licensed Physician, with a Geriatric, Family Medicine, and/or Internal Medicine background, to join their team in Hyannis as their new Senior Health Physician.

About the Opportunity:
- Shift: Day
- Schedule: Four 8-hour shifts per week
- Hours: 32 hours of patient care per week
- Setting: Primary care practice
- Patient Population: Ages 65+
Responsibilities:
- Managing a patient panel with diverse needs
- Conducting acute, chronic, and well visits
- Treating patients in the office and through tele-health
- Participating in continuous learning and clinical rounds
- Collaborating with team members and supervising colleagues
- Performing other duties, as needed
Qualifications:
- 2+ years of recent practice in Primary Care
- Completion of or enrollment in an accredited Residency program
- Board Certified or Board Eligible in Internal or Family Medicine
- State license obtained in Massachusetts
- Ability to communicate with diverse individuals
- Proficiency in using technology for high-quality care
- Strong teamwork skills
Desired Qualifications:
- Advanced Degree in Medicine
- Certification in Geriatric Medicine
- Clinical proficiency in Evidence-based Care
