A community-based healthcare network in New York is seeking a licensed Registered Nurse for a hybrid role. The position focuses on conducting in-home UAS-NY assessments and documenting findings to support accurate care planning and service determination.

About the Opportunity:
- Shift: Day shift
- Schedule: Monday to Friday (occasional weekends)
- Hours: 7:30am to 8pm (flexible; can work up to 10 hours per day; Saturdays = 10am to 6:30pm)
- Setting: Hybrid (3 days field-based / 2 days remote)
Responsibilities:
- Conduct face-to-face in-home UAS-NY assessments within the assigned territory
- Review medical history, medications, and functional status
- Apply UAS criteria to determine level of care and service needs
- Complete assessments and documentation using web-based systems
- Communicate effectively with members, caregivers, and interdisciplinary teams
Qualifications:
- Graduate of an accredited nursing program
- Active New York State RN license
- Minimum 2 years of home-based experience within the last 15 years, including field experience in CHHA, LHCSA, or MLTC settings, assessing homebound individuals, or case management for HCBS, hospice, or assisted living
- Experience conducting assessments similar to UAS, such as OASIS
- Experience in home care as an RN, LPN, or HHA, including private duty nursing
Desired Skills:
- Recent UAS-NY experience within the last 365 days
- Experience working with geriatric populations and chronic disease management
- Familiarity with web-based applications, Microsoft Outlook, Word, Zoom, and virtual assessment tools
- Experience working with diverse and multi-generational populations
- Bilingual (Spanish, Russian, Haitian Creole, or Chinese language skills)
