A health services organization in New York City is actively seeking a licensed Registered Nurse (RN) to join their Assertive Community Treatment (ACT) team in Brooklyn, providing care to adults with serious mental illness across NYC.

About the Opportunity:
- Start Date: ASAP
- Schedule: Monday to Friday (rotating on-call)
- Hours: 9am to 5pm
- Setting: Assertive Community Treatment (ACT) team
- Patient Population: Adult - Mental Illness
Responsibilities:
- Medication management (oral + injectable)
- Health assessments, counseling, psychoeducation
- Coordinating care with providers, shelters, hospitals, etc.
- Traveling across all 5 boroughs for patient visits
- Participating in an on-call rotation
- Supporting/overseeing LPN on the team
- Documentation in AWARDS EMR
Qualifications:
- Graduate of an accredited School of Nursing
- NYS Registered Nurse (RN) license
- Strong psychiatric/behavioral health experience required
- Comfortable with community/mobile work across all boroughs
- Strong assessment and documentation skills
Desired Qualifications:
- Bachelor's Degree in Nursing
- Bilingual (English and Spanish)
- Experience within an Assertive Community Treatment (ACT) team
