A health services network in New York is currently seeking a new Registered Nurse (RN) to join their growing Clinic program in Ulster County.
About the Opportunity:
- Start Date: ASAP
- Schedule: Full Time
- Program: Clinic
- Patient Population: Adults (18+) struggling with addiction, military veterans, homelessness, etc.
Responsibilities:
- Assess the health status of clients assigned to the daily work schedule and develop a plan of care
- Perform and document Nursing Intake/Medical History in accordance with agency policy and procedure.
- Triage sick call list as assigned and make referrals for treatment to the appropriate level of medical practitioner
- Complete follow-up assessment of clients who are treated for chronic and/or acute illnesses to monitor the effectiveness of prescribed treatments
- Screen clients for TB by administering PPD and assessing the reaction to the PPD
- Monitor lab values to identify abnormalities and report such to the attending physician or Physician Assistant
- Reviews medical record to assess care being provided and to ensure continuity of care
- Documents all assessments, interventions, and responses to treatments in the progress notes.
- Provides patient education regarding diet, exercise, medication, and treatments as ordered
- Transcribes medication orders to the medication administration record and completes procedures to obtain medications from the pharmacy
- Takes vital signs as ordered for hypertensive clients, and documents results on the flow sheet
- Maintains inventories of stock items and orders supplies as indicated
- Performs other duties as requested
Qualifications:
- 1+ year of Nursing experience
- Associate's Degree in Nursing
- Computer literacy, including proficiency in Microsoft Office Suite and EHR.
- Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.
- Knowledge of Medical terminology
- Solid assessment, clinical, and documentation skills
- Patient oriented
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
Desired Skills:
- Bachelor's Degree in Nursing
- Experience working with the Substance Abuse and/or Mental Health population
