A health services network in New York City is actively seeking a licensed Psychiatric Nurse Practitioner (PNP) to join their team in Brooklyn, focusing on community-based psychiatric care for the Assertive Community Treatment (ACT) Program.
About the Opportunity:
- Program: Assertive Community Treatment (ACT)
- Setting: Community-Based (Fully Mobile)
Responsibilities:
- Conducting psychiatric evaluations and providing ongoing medication management in community settings
- Providing care to patients in homes, shelters, supportive housing, and public areas
- Participating as a core member of a multidisciplinary ACT team, collaborating with social workers, nurses, case managers, and other providers
- Developing and implementing individualized treatment plans
- Responding to psychiatric crises in the field and coordinating higher levels of care when necessary
- Performing other duties, as needed
Qualifications:
- Master's Degree
- NYS Psychiatric Nurse Practitioner (PNP) license
- DEA registration (for prescribing medications)
- Experience in Community-based Psychiatric programs
- Experience treating patients with SPMI in non-clinical settings (homes, shelters, street outreach)
- Strong crisis management skills
- Ability and willingness to travel within the community to provide care
Desired Qualifications:
- Bilingual
- Prior experience in an Assertive Community Treatment (ACT) program
- Experience working with Homeless or Justice-involved populations
