A strong community health system in New York City is seeking a Primary Care Physician to join their team.
About the Opportunity:
- Schedule: Monday-Friday, 1 Saturday/Sunday per month
- Hours: 9am to 5pm, one later night per week
- Setting: Outpatient
- Patient Population: All ages
Responsibilities:
- Provide comprehensive primary care for patients of all ages in an outpatient community health setting
- Perform routine physicals, preventive care, chronic disease management, and acute visit evaluations
- Support underserved and diverse populations with a focus on access, equity, and continuity of care
- Collaborate with behavioral health, care management, dental, and specialty referral teams
- Maintain accurate documentation and follow quality metrics, compliance standards, and value-based care goals
Qualifications:
- License to practice medicine in New York
- Board Certified/Eligible in Family Medicine
- Interest in community health and working with diverse populations
