A nationwide healthcare organization is seeking licensed Physicians (MD/DO) to perform in-home health evaluations or provide remote collaborating oversight for advanced practice providers. About the Opportunity:
- Setting: In-home patient evaluations or remote collaboration
- Schedule: Flexible; providers set their own hours between 7 AM – 9 PM
- Compensation: Per completed evaluation or collaboration agreement
- Pay Structure: Rates vary by state and evaluation volume (refer to internal pay sheet)

- Arkansas, Arizona, California, Iowa, Illinois, Indiana, Kentucky, Louisiana, Maine, Minnesota, Missouri, Montana, New Mexico, Oklahoma, South Carolina, Tennessee, Virginia, Wisconsin, & West Virginia
Responsibilities:
- Conducting or overseeing comprehensive in-home health assessments
- Evaluating chronic conditions, medications, and preventive care needs
- Reviewing and approving documentation for compliance and quality
- Collaborating with advanced practice providers on patient care plans
- Supporting program quality and data accuracy standards
Qualifications:
- Active MD or DO license in good standing
- Board Certified in Family Medicine, Internal Medicine, or related specialty
- Prior experience with in-home care or population health preferred
- Strong clinical assessment and documentation skills
- Ability to work independently or remotely
- Proficient with electronic documentation systems (iPad/EMR)
Desired Skills:
- Experience with Medicare risk adjustment or value-based care
- Strong communication and collaboration skills
- Flexibility to manage patient evaluations or oversight remotely
