A healthcare network in Morris County, NJ is currently seeking a licensed Physical Therapist (PT) to join their growing team.
About the Opportunity:
- Start Date: ASAP
- Assignment Length: Ongoing (Contract to Hire)
- Schedule: 3 Days Per Week
- Hours: 8am to 5pm
- Patient Population: Adults / Geriatrics
- Patient Volume: 6-10 patients per day
- Setting: Skilled Nursing Facility
- EMR: PCC
Responsibilities:
- Assessment of referred patients that require rehabilitation services including: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
- Establishing and administering a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician.
- Setting realistic and achievable goals for their patients
- Documenting and recording the patient's condition
- Educating patients and families in an appropriate therapy method
- Performing other duties, as needed
Qualifications:
- Graduate of an accredited Occupational Therapy program
- NJ Occupational Therapist (OT) license
Desired Skills:
- Experience with Long-Term Care and/or Geriatric Care
- Well versed with documentation writing
