A healthcare organization in New York City is actively seeking a licensed Nurse Practitioner (NP) to join their team. This role involves providing comprehensive care to Elderplan members within the community, focusing on geriatrics.
About the Opportunity:
- Shift: Day
- Schedule: Monday to Friday
- Hours: Full-time
- Setting: Home Care (Field in the Bronx and Manhattan territories)
Responsibilities:
- Providing comprehensive assessments, including physical and psychosocial evaluations
- Coordinating and developing care plans for geriatric patients
- Delivering primary care services and managing patient health outcomes
- Collaborating with physicians and interdisciplinary teams.
- Ensuring access to preventive care services
Qualifications:
- Minimum of 1 year of clinical experience as a Nurse Practitioner, preferably in geriatrics
- Completion of an approved Nurse Practitioner Program
- New York State RN/NP licenses and DEA certification
- Proficiency in comprehensive patient assessments
- Experience in Care Plan Development
Desired Skills:
- 2+ years of experience in a similar role
- Master's degree in Nursing
- Applicable certifications in Primary Care or Geriatrics
- Fluency in Spanish, Cantonese, or Mandarin
- Previous experience in Managed Care environments
