A nonprofit health organization in New York City is actively seeking a Licensed Practical Nurse (LPN) for a Per Diem position with their growing Mobile Wellness Outreach Team throughout Brooklyn (occasionally the Bronx and Queens).
About the Opportunity:
- Schedule: Consistent set weekday (e.g., Tuesday or Thursday)
- Hours: 8-hour day (7 paid hours, 1 unpaid lunch)
- Setting: Field-based with team support
Responsibilities:
- Providing community-based nursing support
- Conducting wellness checks and engaging high-acuity residents
- Collaborating with team members for comprehensive care
- Supporting residents in medical care plan adherence
- Offering health education and skill-building
- Performing other duties, as needed
Qualifications:
- 2+ years of Nursing experience
- Completion of a Practical Nursing program
- NYS Licensed Practical Nurse (LPN)
- Experience with complex Medical and Behavioral Health conditions
- Proficient with Microsoft Office and electronic health records
- Strong interpersonal skills
Desired Skills:
- Bilingual (English and Spanish)
- Experience in a Supportive Housing role/setting
- Knowledge of working with Homeless or formerly Homeless populations
- Experience in HIV/AIDS care
