LPN – Foster Care Clinic (Article 29-I) – Brooklyn
A leading Child Welfare Agency in Brooklyn is seeking a compassionate LPN to join their Article 29-I Foster Care Health Services Clinic.
About the Opportunity
- Start Date: ASAP
- Schedule: Monday–Friday
- Hours: 9am–5pm or 10am–6pm (10–6 preferred on clinic days)
- Setting: Article 29-I Foster Care Pediatric Clinic
- Patient Population: Children & adolescents in foster care (newborn–21)
- Daily Volume: 6–10 patients per day
- Location: Downtown Brooklyn
Responsibilities
- Provide pediatric clinical care including immunizations, vitals, pre-visit interviews, and room prep/cleanup.
- Maintain updated electronic medical records and ensure all ACS & 29-I documentation requirements are met.
- Conduct physical, mental health, and developmental assessments at admission.
- Arrange initial and annual medical, dental, and vision exams; schedule specialty appointments as needed.
- Manage a caseload of 50–70 youth, coordinating daily appointments, medications, supplies, and follow-ups.
- Develop and maintain care plans and descriptive health plans in collaboration with the medical provider.
- Receive and document visit summaries; make all follow-up appointments and track compliance timelines.
- Provide education to foster parents and youth on preventive health, safety, medications, and age-appropriate guidance.
- Counsel adolescents on HIV prevention, family planning, and STI education; document all discussions.
- Prepare health histories, abstracts, and discharge summaries for transfers or youth aging out of care.
- Collaborate daily with NPs, nurses, therapists, social workers, adoption/home-finding teams, and caseworkers.
- Report illnesses, accidents, and hospitalizations per regulatory guidelines.
Qualifications
- LPN license(NY State)
- 1+ year of nursing experience(no new grads)
- Pediatric experience required; must be comfortable giving immunizations
- Experience with case management or care coordination preferred
- Prior work with foster care, trauma-affected youth, or high-needs pediatric populations a plus
- Proof of vaccinations required
