About the Opportunity:
- Start Date: ASAP
- Schedule: Monday to Friday
- Hours: 8am to 4pm
- Setting: Residential Program
- Patient Population: 12-21 year olds
Responsibilities:
- Attending and accompanying residents to medical appointments
- Preparing necessary paperwork and ensuring follow-up documentation
- Scheduling and confirming medical consults and follow-ups
- Maintaining up-to-date health files and logs
- Communicating appointment results to families and the interdisciplinary team
- Performing other duties, as needed
Qualifications:
- Active NYS Licensed Practical Nurse (LPN) license
- Experience in ID/DD settings
- Familiarity with OPWDD regulations
- Excellent communication skills
- Strong attention to detail
- Compassionate and patient-centered
