A nonprofit services organization in New York City is seeking a Licensed Practical Nurse (LPN) to join their group home in Brooklyn, supporting individuals with intellectual and developmental disabilities.

About the Opportunity:
- Schedule: Monday to Friday
- Hours: 9am to 5pm
- Setting: Group Home
Responsibilities:
- Coordinating medical appointments and follow-up care
- Monitoring residents for medical needs and reporting changes
- Assisting with health services coordination and provider communications
- Providing nursing care during hospitalizations and discharge transitions
- Administering medications and documenting clinical observations
- Performing other duties, as needed
Qualifications:
- Graduate of an accredited School of Nursing
- Active NYS Licensed Practical Nurse (LPN)
- Knowledge of OPWDD regulations
- Strong communication skills
- Problem-solving skills
Desired Qualifications:
- 1+ year of Nursing experience
- Bachelor's Degree in Nursing
- Certification in related fields
- Experience with Intellectual and Developmental Disabilities
