A healthcare organization on Long Island is seeking a Licensed Practical Nurse for a full-time, on-site opportunity in Suffolk County. This role supports patient care in a treatment setting through clinical assessment, medication support, and coordination with the care team.

About the Opportunity:
- Shift: Day
- Schedule: Monday through Saturday or Monday through Sunday, with rotating 8-hour shifts
- Hours: 8-hour shifts; times vary based on rotation
Responsibilities:
- Provide direct nursing care under the supervision of an RN, NP, or physician
- Administer prescribed medications and support self-administration in accordance with scope of practice
- Perform clinical assessments and related nursing procedures, including vital signs, weights, EKGs, wound care, and specimen collection
- Monitor patient condition, document care activities, and report changes to supervising clinicians
- Coordinate follow-up care, referrals, medication records, inventory, and interdisciplinary communication
Qualifications:
- High School Diploma / GED
- Graduate of an accredited Practical Nursing program
- Active NYS Licensed Practical Nurse (LPN)
- Current CPR certification
- Current First Aid certification
- Narcan training
- Infection Control certification
- Child Abuse certification
Desired Qualifications:
- Experience in Behavioral Health, Substance Use Treatment, or Residential Care settings
- EMR proficiency
- Experience with Medication Administration and Clinical Documentation
