A health services organization in New York City is currently seeking a new Licensed Practical Nurse (LPN) for a promising opportunity at one of their group homes in Queens. In this role, the Group Home LPN will be responsible for providing overall medical oversight for program individuals.

About the Opportunity:
- Start Date: ASAP
- Assignment Length: Ongoing
- Hours: 1pm to 8pm
- Patient Population: Medically underserved
Responsibilities:
- Provide observation, monitoring, and documentation for all Individuals served on presenting health needs
- Administer medications and handle the inventory of all medications
- Review the medical data sheets
- Perform medical rounds, nursing procedures, and other treatments as required
- Maintain accurate, neat, concise, and timely documentation in all medical and agency records
- Participate in Interdisciplinary and other health-related meetings as requested
- Assist the Registered Nurse in addressing health teaching needs of the Individuals we serve, their families, and staff
- Perform other duties, as needed
Qualifications:
- Graduate of an accredited School of Nursing
- NYS Licensed Practical Nurse (LPN)
- Experience working with the IDD population
- Solid assessment, clinical, and documentation skills
- Patient oriented
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
