A leading child welfare agency in New York City is actively seeking a Licensed Practical Nurse (LPN) to join one of its Brooklyn clinics.

About the Opportunity:
- Start Date: ASAP
- Schedule: Monday to Friday
- Hours: 9am to 5pm or 10am to 6pm
- Setting: Article 29-I Clinic
- Patient Population Foster Care / Pediatric
- Daily Volume: 6 to 10 patients per day
Responsibilities:
- Provide pediatric clinical care including immunizations, vitals, pre-visit interviews, and room prep/cleanup
- Maintain updated electronic medical records and ensure all ACS & 29-I documentation requirements are met
- Conduct physical, mental health, and developmental assessments at admission
- Arrange initial and annual medical, dental, and vision exams; schedule specialty appointments as needed
- Manage a caseload of 50–70 youth, coordinating daily appointments, medications, supplies, and follow-ups
- Develop and maintain care plans and descriptive health plans in collaboration with the medical provider
- Receive and document visit summaries; make all follow-up appointments and track compliance timelines
- Provide education to foster parents and youth on preventive health, safety, medications, and age-appropriate guidance
- Prepare health histories, abstracts, and discharge summaries for transfers or youth aging out of care
- Report illnesses, accidents, and hospitalizations per regulatory guidelines
- Perform other duties, as needed
Qualifications:
- 1+ year of Nursing experience
- Graduate of an accredited School of Nursing
- NYS Licensed Practical Nurse (LPN)
- Experience working with the Pediatric population
Desired Skills:
- Experience with Case Management and/or Care Coordination
- Prior work experience with Foster Care, Trauma-affected Youth, or high-needs Pediatric populations
