Social service agency is seeking a Licensed Practical Nurse to join their team.
About the Opportunity:
- Shift: Nights
- Schedule: 1 shift per week
- Hours: 8pm-8am
- Setting: Homeless shelter
Responsibilities included but not limited to are
:
- Management of medication system for all clients at shelter, including
- Receiving and reviewing medications
- Assists with calling for refills
- Assuring safe storage of medications
- Ensuring that clients have appropriate supplies and access to medications
- Monitoring client reactions for side effects and reporting to prescribing provider
- Keeping Medication Self Administration logs, destruction log, controlled substance log
- Provides nursing screening of all new clients
- Assists in connecting psychiatric, medical and substance abuse problems to appropriate providers
- Participates in clinical meetings with medical staff, shelter case management, & administrative staff
- Communicates regularly with on-site psychiatric and medical providers, case managers and on-site operations & administrative staff
- Communicates with off-site psychiatric providers as necessary and appropriate
- Administers medications ordered by the providers
- Administers injections ordered by providers
- Provides medication teaching as needed
- Assists clients with self-administer medications
- Provides ongoing health education and support to clients
- Maintains documentation in compliance with agency policies and procedures
- Assist with tele-psychiatry operations as scheduled with tele-psychiatry provider
- Assist with Primary Care Clinic, Residential, Substance Use Program coverage as needed
- Utilizes Electronic Medical Records system
Requirements:
- Active New York State Licensed Practical Nurse license
- American Heart Association (AHA) BLS certification
- 3 years of clinical experience working as a nurse with the mental health or substance abuse population
- Experience working with an Electronic Medical Records (EMR) system
- Must have knowledge Microsoft Office, especially Outlook, Word and Excel.
- Must have excellent oral and listening skills
- Must have the ability to interface with clients as well as all levels of staff.
Preferred :
- Working with homeless individuals preferred
