A social services organization in New York City is currently seeking a licensed Licensed Practical Nurse (LPN) to join their growing Forensic Assertive Community Treatment (FACT) team in Queens.
About the Opportunity:
- Start Date: ASAP
- Programs: Forensic Assertive Community Treatment (FACT)
- Schedule: Monday to Friday
- Hours: 9am to 5pm (21 hours per week - flexible)
- Caseload: 5 per day
- EMR: Awards
Responsibilities:
- Conducting mental status exams
- Assessing physical health needs
- Making appropriate referrals to Community Physicians
- Providing management and administration of medications in conjunction with a Psychiatrist
- Participating on team that delivers an integrated array of treatment, rehabilitation and support services, customized to each person
- Working effectively in conjunction with the psychiatrist and registered nurse, as well as others on the treatment team for persons with substance abuse disorders
- Perform other related duties, as assigned
Qualifications
- 1+ year of experience in a Treatment and/or Rehabilitation setting
- Graduate of an accredited School of Nursing
- NYS Licensed Practical Nurse (LPN) license
- Knowledge of Nursing assessment methods
- Solid assessment, clinical, and documentation skills
- Patient oriented
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
