A health services organization in Virginia is seeking a new Licensed Practical Nurse (LPN) to join their team at a Crisis Therapeutic Home, working with adults needing crisis prevention and stabilization services.
About the Opportunity:
- Shift: Evening or Overnight
- Schedule: Monday to Friday (with rotating weekends)
- Hours: 1pm to 11pm or 11pm to 7am
- Setting: Crisis Therapeutic Home
Responsibilities:
- Implementing nursing and care management services
- Creating and maintaining Individual Service Plans
- Conducting physical and mental assessments
- Monitoring consumer safety and medication compliance
- Coordinating care with healthcare providers
- Performing other duties, as needed
Qualifications:
- 2+ years of experience in a Mental Health or ID/DD setting
- Graduation from an accredited Practical Nurse Program
- VA Licensed Practical Nurse (LPN)
- Experience with Electronic Medical Records
- Understanding of Medicaid/Medicare
- Proficiency in Microsoft Office
- Strong communication skills
Desired Qualifications:
- Advanced certifications in Mental Health
- Experience in Mental Health Care
- Strong clinical skills in Psychiatric Care
- State Hospital and/or Acute Care experience
