A health services organization in Westchester County, New York is seeking a new Intake Nurse or Social Worker to conduct community-based assessments for Medicaid patients. This role is focused on non-clinical assessments, with no direct patient care involved.
About the Opportunity:
- Schedule: Part-Time (minimum 2 days per week)
- Hours: 20-25 hours/week
- Setting: Community and office-based
Responsibilities:
- Traveling to patients’ homes to conduct scheduled intake assessments
- Completing standardized medical and functional evaluations
- Determining care needs and eligibility for services
- Documenting findings accurately and promptly
- Supporting intake prioritization based on urgency
- Performing other duties, as needed
Qualifications:
- 1+ year of relevant experience in UAS and assessments
- Bachelor's Degree in Nursing or Social Work
- Applicable certifications in UAS or related fields
- Strong assessment skills

Desired Qualifications:
- Master's Degree in a Healthcare-related field
