- Shift: Day Shift
- Schedule: Monday to Friday
- Hours: Full-time
- Setting: Home care (Manhattan and Bronx territory)
Responsibilities:
The Nurse Practitioner will be responsible for:
- Providing comprehensive assessments, including physical and psychosocial evaluations.
- Coordinating and developing care plans for geriatric patients.
- Delivering primary care services and managing patient health outcomes.
- Collaborating with physicians and interdisciplinary teams.
- Ensuring access to preventive care services.
Qualifications:
- Minimum of 1 year of clinical experience as a Nurse Practitioner, preferably in geriatrics
- Master's degree in Nursing and completion of an approved Nurse Practitioner Program
- New York State RN/NP licenses and DEA certification
- Proficiency in comprehensive patient assessments
- Experience in care plan development
- Strong ability to coordinate with healthcare teams
- Excellent communication skills
- Empathy and compassion in patient care
- Strong organizational skills
Desired Skills:
- 2+ years of experience in a similar role
- Master's degree in Nursing
- Applicable certifications in primary care or geriatrics
- Fluency in Spanish, Cantonese, or Mandarin
- Previous experience in managed care environments
