About the Opportunity:
- Shift: Day
- Schedule: Monday to Friday, with a possible 4-day work week
- Hours: Business hours
- Setting: Federally Qualified Health Center (FQHC)
Responsibilities:
- Providing comprehensive primary care to a diverse patient population
- Managing both adult and pediatric patients
- Collaborating with a multidisciplinary team
- Adapting care plans based on patient needs and feedback
- Participating in community health initiatives
- Performing other duties, as needed
Qualifications:
- Master's Degree
- NYS Family Nurse Practitioner (FNP) license
- Experience in Primary Care settings
- Ability to handle a diverse patient demographic
- Proficiency in Electronic Medical Records (EMRs)
- Strong communication skills
- Empathy and cultural competence
- Problem-solving abilities
Desired Qualifications:
- Bilingual (English and Spanish)
- Experience in an FQHC setting
- Experience with HIV Care
