A health services network in New York City is actively seeking a new Case Worker to join their growing Social Department in the Bronx.
About the Opportunity:
- Schedule: Monday to Friday
- Hours: 8am to 4pm or 8:30am to 4:30pm (35-hour work week with 1-hour unpaid break)
- Setting: Acute Care Hospital
Responsibilities:
- Conducting interviews, initial assessments, and referrals
- Managing cases of social service recipients
- Assessing patient needs
- Investigating and verifying information
- Providing health-related and social services to clients
- Performing other duties, as needed
Qualifications:
- 3 years of related Social Work experience
- Associate's Degree
- Proficiency in Case Management
- Experience with Substance Use Disorder populations
- Computer savvy
- Strong interpersonal and communication skills
- Strong attention to detail
- Highly organized
Desired Qualifications:
- Bachelor's Degree
- Bilingual
- Solid understanding of complex Case Management
- EPIC EMR proficiency
