About the Opportunity:
- Shift: Day
- Schedule: Monday to Friday
- Hours: 9 am to 5 pm (35-hour work week)
- Setting: Acute Care Hospital
Responsibilities:
- Conducting interviews, initial assessments, and referrals
- Managing cases of social service recipients
- Working directly with patients to assess their needs
- Investigating and verifying gathered information
- Providing health-related and social services to clients
Qualifications:
- 3 years of experience in social work
- Bachelor's Degree
- Familiarity with EPIC EMR system
- Experience in case management
- Proficiency in entering and managing data
- Strong assessment skills
- Excellent communication skills
- Ability to work in a team
- Attention to detail
Desired Skills:
