A New York City healthcare organization is seeking a Case Planner to join their team in Brooklyn, supporting the Specialized Family Foster Care program.
About the Opportunity:
- Start Date: ASAP
- Schedule: Monday to Friday
- Hours: 8am to 4pm or 9am to 5pm or 10am to 6pm or 11am to 7pm
Responsibilities:
- Creating service plans with children and families
- Monitoring safety and progress through regular contact
- Scheduling and supervising family visits
- Attending Family Court hearings and providing testimony
- Participating in Family Team Conferences
- Performing other duties, as needed
Qualifications:
- 1 year of Case Management experience
- Bachelor's Degree
- Computer savvy
- Proficiency in Microsoft Office
- Great interpersonal and communications skills
Desired Qualifications:
- Master's Degree
- Bilingual (English and Spanish)
- Experience in Child Welfare and Foster Care
- Knowledge of the Connections system
