A community services organization in New York City is seeking a new Case Planner to join their team working on Staten Island and Queens, providing care and support to foster youth in a residential setting.

About the Opportunity:
- Schedule: Monday to Friday
- Hours: 9am to 5pm
- Setting: Hybrid (Office/Field Based)
Responsibilities:
- Managing a caseload of 12 foster youths
- Coordinating with team members including Supervisors and Care Staff
- Attending court hearings and field activities
- Utilizing ACS - Connections EMR system
- Providing comprehensive support to youth with diverse backgrounds
Qualifications:
- 1+ year of Case Planning experience
- Education: Bachelor’s Degree
- Computer Savvy
- Strong communication skills
- Ability to work in a team dynamic
Desired Qualifications:
- Experience with ACS - Connections
- Experience with social work, foster care, or juvenile justice
- Familiarity with youth residential programs
- Conflict resolution skills
