A transitional housing organization is seeking a Case Manager in Brooklyn, NY to provide direct support to individuals experiencing homelessness and help them transition to permanent housing. The role focuses on case management, housing placement support, benefits coordination, and connection to behavioral health and medical services.
About the Opportunity:
- Shift: Day shift
- Schedule: Monday-Friday
- Hours: 9:00am-5:00pm, with one late day from 11:00am-7:00pm
- Setting: In-person, shelter setting
Responsibilities:
- Provide direct case management services to a caseload of approximately 15 clients
- Conduct assessments, psychosocial evaluations, and weekly client check-ins to support service planning
- Coordinate connections to psychiatric care, medical treatment, substance use treatment, and other supportive services
- Assist clients with obtaining identification, public benefits, income sources, and permanent housing
- Maintain timely documentation and records in compliance with agency, city, and state standards
Qualifications:
- Bachelor’s degree
- At least 2 years of experience working with homeless adults and substance use issues
- Experience using harm reduction and motivational interviewing techniques
- Excellent computer skills, including Microsoft Word and Excel
- Strong writing and communication skills
Desired Skills:

- Experience working with clients with mental health needs
- Bilingual Spanish/English skills
