A nonprofit services organization in New York City is currently seeking a Case Manager for a transitional housing role with their team. In this role, the Case Manager will be responsible for dealing with clients facing homelessness and potentially mental health or substance abuse issues.
About the Opportunity:
- Schedule: Monday to Friday
- Hours: 9am to 5pm (one late night a week =11am to 7pm)
- Setting: Transitional Housing
Responsibilities:
- Providing direct services to a caseload of approximately 15 clients
- Conducting psychosocial evaluations
- Connecting clients to psychiatric and medical services
- Obtaining identification and benefits for clients
- Meeting weekly with clients to develop service plans
- Performing other duties, as needed
Qualifications:
- Bachelor’s Degree
- Experience in a Shelter, Mental Health, or Substance Abuse setting
- Experience with Harm Reduction techniques
- Motivational interviewing skills
- Proficient in Microsoft Word and Excel
- Excellent writing and communication skills
Desired Qualifications:
- Bilingual (English and Spanish)
- Experience with AWARDS/Cares systems
- Motivational Interviewing techniques
