A Long Island-based healthcare organization is seeking a licensed Registered Nurse (RN) to join their team in Nassau County as a Case Manager, focusing on coordinating high-quality, cost-effective care for hospitalized patients.

About the Opportunity:
- Schedule: Monday to Friday
- Hours: 8:30am to 5:30pm
Responsibilities:
- Managing inpatient care and discharge planning
- Performing admission and concurrent reviews
- Coordinating interdisciplinary care teams
- Developing effective discharge plans
- Ensuring compliance with healthcare regulations
Qualifications:
- 2-3+ years of managed care experience
- Graduate of an accredited School of Nursing
- Active NYS Registered Nurse (RN) license
- Strong understanding of Managed Care workflows
- Experience with Utilization Review and Discharge Planning
- Strong communication and organizational skills
- Critical thinking and problem-solving abilities
- Ability to work independently and manage priorities
Desired Qualifications:
- Bachelor's Degree in Nursing
- Knowledge of NCQA standards
