A health services network in New York City is seeking a licensed Registered Nurse (RN) to join their team in the Bronx as a Case Manager.
About the Opportunity:
- Schedule: Monday to Friday
- Hours: 10am to 6pm
- Setting: Acute Care
Responsibilities:
- Utilization management, Patient Review Instrument (PRI), and discharge planning
- Coordinating and managing patient care from admission to discharge
- Developing and implementing care plans and facilitating referrals
- Conducting comprehensive assessments and adjusting care plans as needed
- Facilitating communication among patients, families, and healthcare providers
Qualifications:
- 5 years of Clinical experience as a Registered Nurse
- Associate’s Degree (ASN, ADN) from an accredited Nursing Program
- RN License in State of New York
- PRI and Screen certification
- BLS certification
- Strong computer literacy
- Excellent communication skills
- Problem-solving abilities
- Team collaboration skills
Desired Skills:
- 3+ years Care Management experience
- Experience with EPIC EMR system
- Additional certifications in Care Management
- Effective communication with diverse populations
