A health services organization on Long Island is actively seeking a licensed Registered Nurse (RN) to join their team in Nassau County as a Case Manager, managing high-risk members who require enhanced intervention.

About the Opportunity:
- Shift: Day
- Schedule: Monday to Friday
- Hours: 8:30am to 5:30pm (1-hour unpaid lunch, flexibility of 9 AM to 5 PM during TEMP period)
Responsibilities:
- Providing intensive care management and coordination for high-risk members
- Collaborating with interdisciplinary teams for comprehensive care plans
- Engaging in the transition of care management between various settings
- Ensuring timely access to appropriate care services
- Maintaining accurate documentation in care management systems
- Performing other duties, as needed
Qualifications:
- 2–3+ years of experience in a Managed Care setting
- Graduate of an accredited School of Nursing
- NYS Registered Nurse (RN) license
- Proficiency in clinical assessment and critical thinking
- Ability to manage complex cases independently
- Proficiency in Microsoft Office applications
- Organizational and time management skills
- Excellent written and verbal communication skills
- Interdisciplinary team collaboration skills
Desired Qualifications:
- Minimum 2 years of Insurance, MSO, or Care Management experience
- Bachelor's Degree in Nursing
- Case Management Certification (CCM or equivalent) preferred
- Knowledge of Home Health and Post-Acute Care Services
- Experience working with High-Risk and Geriatric populations
