A dedicated healthcare organization in New York City is currently seeking a licensed Registered Nurse (RN) for a great Case Manager position with their team in Brooklyn and Queens.
About the Opportunity:
- Schedule: Part-time and Full-time available!
- Hours: 9 am to 5 pm
- Setting: Field across designated sites (Brooklyn & Queens)
Responsibilities:
- Coordinating delivery of routine and specialty healthcare services
- Assessing and establishing healthcare outcomes
- Acting as a liaison to communicate health information
- Completing required documentation and charting healthcare progress
- Conducting health-related training for staff and individuals receiving service
Qualifications:
- 1+ year of nursing experience
- Graduate of an accredited School of Nursing
- NYS Registered Nurse (RN) license
- Strong clinical skills
- Ability to travel between multiple sites
- Flexible with working hours
- Excellent communication skills
- Ability to work in compliance with all healthcare regulations
- Enthusiastic and dedicated to making a positive impact
Desired Skills:
- Bachelor's Degree in Nursing
- Experience with the IDD population
- Experience in a community or field-work setting
- Proficiency with EMR systems
