A nonprofit services organization in New York City is seeking a Licensed Practical Nurse (LPN) to join their Preventive Services Program as a Case Manager.
About the Opportunity:- Schedule: Full-time
- Hours: 9am to 5pm or 10am to 6pm or 11am to 7pm
- Conducting family-based home visits
- Assessing health concerns and providing education/support
- Helping families understand medical recommendations and follow-up care
- Serving as a liaison between families, hospitals, clinics, and community providers
- Coordinating care with social workers, program leadership, and medical providers
- Supporting preventive health, chronic illness management, maternal/child health, and family health education
- Completing documentation and follow-up as needed
- Graduate of an accredited School of Practical Nursing
- Active NYS Licensed Practical Nurse (LPN)
- Solid assessment, clinical, and documentation skills
