This position is for a Case Manager located in New York, NY, responsible for managing a caseload of participants and developing comprehensive service plans.
About the Opportunity:
- Shift: Monday – Friday
- Schedule: 8 AM – 5 PM
- Hours: Full-time
- Setting: Adults with mental and physical disabilities
Responsibilities:
The Case Manager will be responsible for:
- Handling intakes
- Scheduling follow-ups
- Conducting case conferences
- Documentation
- Setting up resources and wellness services
Qualifications:
- Case Management experience
- Associate's Degree
- Basic Microsoft Office proficiency
- Excellent verbal and written communication skills
- Highly organized
- Attention to detail
- Patience with individuals with disabilities
- Interpersonal skills
- Ability to follow agency policies
