An established social services organization is seeking a Case Manager in Brooklyn, NY. This role supports residents through assessment, advocacy, service coordination, and ongoing case management in a residential setting.

 About the Opportunity:
  • Shift: Day shift
  • Schedule: Monday through Friday, with a Saturday rotation and one late day per week
  • Hours: Standard business hours, with occasional late-day coverage and summer Fridays ending at 3:00 PM as workload permits
  • Setting: Residential setting
Responsibilities:
  • Manage a caseload of residents and meet with clients on a weekly and as-needed basis
  • Connect clients with appropriate providers, programs, and community resources across health and human services settings
  • Escort clients to scheduled and unscheduled medical and psychiatric appointments when needed
  • Facilitate group activities, track participation, and support on- and off-site programming
  • Maintain accurate client records, including progress notes, service plans, assessments, and updates, in an audit-ready format
  • Conduct unit inspections, support follow-up needs, and ensure safe service delivery in alignment with program and food safety standards
Qualifications:
  • Bachelor’s degree
  • At least 2 years of experience in human services
  • Strong spoken and written communication skills
  • Strong time management skills and the ability to multitask in a fast-paced environment
  • Ability to build therapeutic relationships with clients and collaborate with internal and external partners
  • Proficiency with Microsoft Office, email, and internet navigation
  • Knowledge of confidentiality and HIPAA-related practices
Desired Skills:
  • Background in case management
  • Experience supporting individuals affected by homelessness, HIV, substance use, mental illness, or entitlement-related needs
  • Experience with crisis intervention
  • Flexibility to work early or late shifts, weekends, and holidays as needed
  • Familiarity with ongoing training and continuing education in human services
