A New York City health services network is seeking a dedicated professional to join their Brooklyn team as a Case Manager. In this role, the Case Manager will be responsible for focusing on case management for adolescents with mental health issues.
About the Opportunity:
- Schedule: Monday to Friday
- Hours: 10am to 6pm
- Setting: Field / Office work
Responsibilities:
- Interviewing and engaging with youth upon arrival
- Providing ongoing case management, including treatment team and school meetings
- Coordinating with mental health providers and managing family engagement
- Ensuring field work is completed as per youth service plans
- Creating and documenting treatment plans
- Performing other duties, as needed
Qualifications:
- 1+ year of relevant work experience
- Bachelor's Degree
- First Aid certification within 6 months
Desired Qualifications:
- Experience in Trauma-focused interventions
