A health organization in New York City is actively seeking a new Case Manager in Brooklyn, focusing on preparing youth for independent living within the area.

About the Opportunity:
- Schedule: Monday to Friday
- Hours: 9am to 5pm or 10am to 6pm or 11am to 7pm or 12pm to 8pm
- Setting: Field locations and occasional office work
Responsibilities:
- Developing individual case service plans according to guidelines
- Determining and coordinating necessary services for clients
- Scheduling appointments and escorting clients to resources
- Enrolling clients in appropriate schools and programs
- Monitoring service delivery and client progress
Qualifications:
- 3+ years of relevant experience
- Bachelor’s Degree in Human Services or related field
- Experience in Case Management and Field Work
- Ability to travel within NYC
- Strong verbal and written communication skills
- Organizational and coordination skills
- Interpersonal skills for diverse populations
Desired Skills:
- Master’s Degree in Social Work
- Knowledge of ACS Connections
- Experience with Child Welfare and Supportive Housing
