A nonprofit organization is seeking a Social Worker in Brooklyn, NY to supports older adults (60+) to age in place by delivering case management, benefits/entitlements assistance, health-related support in partnership with VNS, translation services, support groups, educational and recreational activities, and initiatives to reduce social isolation.
About the Opportunity:
- Schedule: Monday to Friday
- Hours: 9am to 5pm
Responsibilities:
- Conduct intake and annual assessments; manage a steady caseload of ongoing case management clients
- Provide case assistance for one‑time needs (forms, benefits inquiries, photocopying, referrals)
- Coordinate health‑related support with VNS (e.g., routine blood pressure checks)
- Reduce social isolation through engagement in groups, educational/recreational activities, and community touchpoints
- Enter clear, concise notes and units of service in the Vive database (training provided)
- Maintain audit‑ready records aligned with NYC Department for the Aging requirements
- Complete daily/weekly worker logs and reconcile activity to services delivered
- Collaborate closely with a supportive, multilingual office; share information to keep staff aligned
- Assist with light program operations (e.g., setting up rooms, serving refreshments) for events and groups
- Uphold client‑centered, empathetic service; escalate complex family/health issues appropriately
Qualifications:
- High School Diploma / GED
- Demonstrated attention to detail and reliability in documentation under audit standards
- Strong written communication
- Comfort working with older adults; empathetic, professional demeanor.
Desired Skills:
- Bachelor’s degree (BSW/Human Services or any field).
- Familiarity with prior systems (SARS / Peer Place).
- Russian language skills (significant plus given community demographics)
- Experience in benefits/entitlements assistance, case management, or social services
