A nonprofit organization in New York City is actively seeking a new Case Manager for a promising opportunity with their growing team.
Responsibilities:
- Conduct intake assessments and develop independent living plans with families
- Provide comprehensive case management services to help families achieve housing stability
- Connect clients to community resources, benefits, and permanent housing opportunities
- Monitor client progress through regular face-to-face sessions and maintain documentation in the database
- Advocate for clients with external organizations regarding housing, healthcare, education, and legal matters
- May escort clients to appointments and assist with benefit applications
Qualifications:
- Bachelor's Degree in a related field and/or 2+ years of relevant Case Management experience
- Family Shelter Operations and/or Case Management experience working with families in crisis
- Strong understanding of Family Shelter environments and commitment to supporting Homeless families
- Ability to work effectively with diverse populations
- Must pass background clearances (SCR, SEL, fingerprinting)
Desired Skills:
- Bilingual (English and Spanish)
