A New York City-based health services organization is seeking a new Case Manager to join an Assertive Community Treatment (ACT) Team in Queens, providing intensive mental health services to individuals with severe and persistent mental illness.

About the Opportunity:
- Start Date: ASAP
- Schedule: Monday to Friday
- Hours: 9am to 5pm
- Setting: Hybrid (3-4 days fieldwork, 1-2 days remote)
Responsibilities:
- Providing direct, community-based services to clients
- Developing and managing individualized treatment plans
- Assisting with daily living skills and social integration
- Coordinating care with healthcare and social service providers
- Monitoring progress and documenting services
- Performing other duties, as needed
Qualifications:
- Minimum of 6 months of experience with mental illness
- Bachelor’s Degree in Social Work, Psychology, Counseling, or related field
- Knowledge of Community Resources
- Strong interpersonal and communication skills
- Strong attention to detail
- Highly organized
- Ability to work independently
Desired Qualifications:
- Master's Degree
- Experience working within an Assertive Community Treatment (ACT) or an Intensive program
- Bilingual (English and Spanish)
- Familiarity with EHR systems
