A New York City-based health services organization is seeking a new Case Manager to join an Assertive Community Treatment (ACT) Team in Queens, providing intensive mental health services to individuals with severe and persistent mental illness.

 About the Opportunity:
  • Start Date: ASAP
  • Schedule: Monday to Friday
  • Hours: 9am to 5pm
  • Setting: Hybrid (3-4 days fieldwork, 1-2 days remote)
Responsibilities:
  • Providing direct, community-based services to clients
  • Developing and managing individualized treatment plans
  • Assisting with daily living skills and social integration
  • Coordinating care with healthcare and social service providers
  • Monitoring progress and documenting services
  • Performing other duties, as needed
Qualifications:
  • Minimum of 6 months of experience with mental illness
  • Bachelor’s Degree in Social Work, Psychology, Counseling, or related field
  • Knowledge of Community Resources
  • Strong interpersonal and communication skills
  • Strong attention to detail
  • Highly organized
  • Ability to work independently
Desired Qualifications:
  • Master's Degree
  • Experience working within an Assertive Community Treatment (ACT) or an Intensive program
  • Bilingual (English and Spanish)
  • Familiarity with EHR systems
