A health services organization on Long Island is seeking a Care Coordinator for a promising opportunity with their team in Suffolk County, working with Medicaid clients to address social determinants of health through fieldwork and community engagement.

About the Opportunity:
- Shift: Daytime
- Schedule: Monday to Friday
- Hours: 9am to 5pm
- Setting: Primarily Remote (with occasional fieldwork)
Responsibilities:
- Screening Medicaid clients for social determinants of health
- Determining client eligibility for community services
- Conducting cold-call outreach to clients
- Documenting interactions using Excel and UNITUS platform
- Coordinating client follow-ups to ensure service access
- Performing other duties, as needed
Qualifications:
- 1-2 years of relevant experience
- High School Diploma / GED
- Proficiency in Excel and documentation tools
- Experience with Medicaid populations
- Comfortable working independently
- Comfort with cold-calling and outreach
- Strong interpersonal and communication skills
Desired Qualifications:
- 2+ years of experience in Community Health roles
- Associate's and/or Bachelor's Degree
- Experience in Health Home Worker or Community Health Worker roles
- Proficiency with the UNITUS platform
